The Medical Social Worker (MSW) at Tri-Valley Home Health Care is essential in supporting patients and their families by addressing the social, emotional, and environmental factors that influence recovery and well-being. The MSW collaborates with our interdisciplinary healthcare team to develop comprehensive care plans that integrate medical and social care, ensuring holistic support for patients in the comfort of their own homes.

  • Assessment of Patient Needs: Conduct initial evaluations to assess the patient’s and family’s social, emotional, and environmental needs and develop a tailored care plan in collaboration with the healthcare team.
  •  Emotional Support and Counseling: Provide emotional support, grief counseling, and mental health resources to patients and their families to help them cope with illness, injury, or life transitions.
  • Resource Coordination: Connect patients with community resources such as financial assistance, housing modifications, transportation, or support groups. Advocate on behalf of patients to secure necessary services.
  • Crisis Intervention: Offer immediate support and interventions during emotional or health crises, coordinating with mental health professionals and other services as necessary.

Caregiver Support: Educate and support family caregivers, helping them manage their duties and providing resources such as respite care.

  •  Discharge Planning: Assist with care transitions from home health to other care settings (e.g., rehabilitation, nursing homes, hospice) or help plan discharges and follow-up care as needed.
  • Legal and Ethical Guidance: Assist patients with advance directives and healthcare proxies and ensure patient rights are respected throughout the care process.
  • Collaboration: Work closely with nurses, therapists, and doctors to provide comprehensive, patient-centered care.
